This is the first step in the screening process in order to work with Children or Youth at First United Methodist Church of Plant City.  You may fill out the form below or print the entire form (PDF below) and send or drop it off at the Church.

NOTE:  After filling out the online form, it is still required that you print out the SIGNATURE PAGE and both APPENDIX (IV and V), hand-write your signature, and send or drop them off to either the Children's Department or Youth Department.
